    • Execution of treatment plan in a stepwise manner, with complexity of care varying according to the risk of developing chronic pain and disability.

    • A single point of contact for coordination of care including comprehensive progress reports at regular intervals.

Chronic condition (> 3 months)

  • Improve chronic pain and prevent permanent disability. 

  • Approach:

    • Assessment by pain physician (virtual or in-person).

    • Development of an individualized treatment plan that is grounded in modern pain science.

    • Execution of treatment plan in a stepwise manner. Emphasis on integrated, multidisciplinary care.

    • A single point of contact for coordination of care including comprehensive progress reports at regular intervals.
